She appears to be getting off on the fact that he has a really, really cold pick.
Or she’s some horrible monster sealed in the mine, and the miner is going to need that last bullet after she’s no-sold all the other bullets and killed everyone else standing between them.
I’m reminded of the classic, if terrible, “take your pick” joke. This guy literally took his pick.
He’s just finishing the final practice swing for the (literally) underground extreme nipple piercing competition.

I am aware of what the title says. “Save the last bullet for yourself” is an inside joke for weird fiction fans inspired by the title.
